Dr. Toriumi is a Rhinoplasty Expert

Because every nose is different and its anatomy varies from patient to patient, rhinoplasty is one of the most difficult operations in facial plastic surgery. As the nasal anatomy varies, so must the techniques used for correction.

A successful rhinoplasty requires:

  • A precise and complete nasal analysis
  • Using a nasal analysis to spot all of the aesthetic and functional deformities
  • A surgical plan to correct the specific deformities

It takes a great deal of experience to accomplish these steps. Dr. Toriumi's rhinoplasty practice has encountered the many different combinations of nasal anatomy. For this reason, Dr. Toriumi can effectively correct the problems at hand.

Common problems such as a bulbous nasal tip and a small dorsal hump are less complicated to correct compared to a previously operated or severely over-resected nose. Even less complicated nasal surgeries require attention to detail and precision.

Dr. Toriumi Gives You the Best Outcome

Dr. Toriumi understands that rhinoplasty is a very difficult operation and he does everything possible to provide you with the best outcome. For this reason, he takes all the necessary time to correct all deformities to his satisfaction.

Dr. Toriumi spends 3 to 4 hours to complete a typical primary rhinoplasty. A moderately complex secondary case takes him 5 to 6 hours. Extra operative time tending to fine details often means it will be more expensive to have surgery with Dr. Toriumi compared to a typical surgeon. Dr. Toriumi does his best to give you the best outcome that he can. After all, his intention is to make it your last rhinoplasty operation.

Dr. Toriumi follows his patients over many years and will work closely with you to maximize your long-term result. Long-term follow-up is critical to insuring a good result and Dr. Toriumi encourages very regular and long term postoperative consultations. He believes that noses must be made strong to resist the forces of scar contracture that occur over time. At one year, for example, your nose is only about 50% to 60% healed with the remainder of the healing occurring over your lifetime.

Keeping your appointments maximizes your postoperative outcome. Dr. Toriumi may frequently ask you to do nasal exercises, protect your nasal structure with tape, and receive occasional steroid injections. These postoperative techniques can have a major impact on your long term outcome.

Dr. Toriumi Believes in No Standard Aesthetic

Dr. Toriumi does not adhere to one aesthetic goal in rhinoplasty.  He listens to each patient’s request and works with each case individually using 3-D computer imaging to be certain he is clear about what you want and to come to an agreement.  He prefers that your nose look good from the front as this is what you see in the mirror and the view others see as well.

Dr. Toriumi also feels it is important to have your nose balance with other facial structures.  If you have a weak chin, for example, he may suggest a chin augmentation to help balance your nose.

Dr. Toriumi Specializes in Complex Reconstructions

Secondary Rhinoplasty

A good portion of Dr. Toriumi's practice is devoted to secondary rhinoplasty (correcting problems from a previous rhinoplasty).

For most secondary rhinoplasty patients, Dr. Toriumi must construct a new supporting nasal structure to provide a proper shape. Such major reconstructions are very difficult and require an accurate understanding of three-dimensional nasal contours.

When a patient undergoes a secondary rhinoplasty procedure, scar tissues develop that can complicate subsequent reconstructions. Dr. Toriumi understands there is tremendous variability in secondary rhinoplasty deformities. Some patients have problems that are difficult to correct. This is why some noses can be effectively corrected and others carry a higher risk of problems or persistent deformity.

There are a number of complications that are very common after rhinoplasty. Dr. Toriumi has developed a rhinoplasty technique that will aid in correcting these common complications:

  • Collapse of the lateral wall
  • Pinched or overnarrowed nasal tip
  • Pinched middle nasal vault

Dr. Toriumi Emphasizes Preservation with Minimal Excision

Dr. Toriumi has created rhinoplasty techniques that enhance the preservation of your nasal structures with minimal excision of supporting tissues.  He avoids excessive removal of supporting structures because it can result in a pinched, upturned tip with nasal obstruction – an “operated look”.  Dr. Toriumi may use cartilage grafts taken from behind your ear or from rib to increase the strength of the nose. This minimizes undesirable changes that can otherwise occur.

Dr. Toriumi Maximizes Your Nasal Function

Dr. Toriumi believes there is no need to compromise your nasal function for a better looking nose. Your nasal function is critical and he considers it as important as your aesthetic outcome. In fact, after surgery you will breathe better and have improved nasal function over the long term. Even if you deny nasal obstruction, Dr. Toriumi will maximize your nasal airway to avoid nasal obstruction as you age.
